Tesamorelin is a lab-made, complete copy of the natural hormone that tells your pituitary gland to release growth hormone (GHRH), with a small chemical tag attached to the front to help it last longer in the body. It's sold under the brand names EGRIFTA, EGRIFTA SV, and EGRIFTA WR (FDA label).
Mechanism under study
It works the same way as Sermorelin (see that entry) — attaching to the pituitary gland's receptor and telling it to release the body's own natural growth hormone in a normal, pulsing pattern, rather than injecting growth hormone directly. This growth hormone then raises another hormone called IGF-1, which is responsible for many of the downstream effects on fat and muscle. Unlike some other hormone-boosting drugs, it doesn't meaningfully raise stress hormones or other unrelated hormones.
Human evidence summary
This is one of the most well-studied peptides in this category, with large official drug-approval trials involving over patients over 26 weeks. In a separate 6-month study, it reduced visceral fat by an average of 34 square centimeters more than a placebo and reduced fat stored in the liver as well (Stanley et al.). Blood sugar went up slightly at 2 weeks but the difference wasn't significant by 6 months. Importantly, it is not approved and not well-studied for general weight loss or bodybuilding use in people without HIV-related fat problems.
Preclinical evidence summary
The existing profile data does not report dedicated animal studies for Tesamorelin — its evidence base comes almost entirely from human clinical trials, since it went through the full official drug-approval process.
