Research overview

Semax is a synthetic peptide analog derived from a fragment of adrenocorticotropic hormone. Research has examined neuroprotective signaling, cognition-related pathways, stress responses, and neurological recovery models.

Much of the clinical literature originates outside the United States, while many mechanistic questions remain the subject of ongoing research.

Human evidence summary

In Russia, Semax has real clinical use for stroke, "mini-strokes" (transient ischemic attacks), certain eye-nerve disorders, and cognitive problems. There's also a brain-scanning (fMRI) study looking at how Semax (and Selank) affect connections between brain regions (Dokl Biol Sci 2020), and a broader review summarizing this Russian research (review). The important caveat: these studies are small, come from one country/research network, weren't always run to the strict standards used in the West, and have never been independently confirmed by outside scientists. Using it just to boost focus in healthy people is based on personal reports, not real trials.

Preclinical evidence summary

This is where the strongest evidence lies. In rodent studies, Semax reliably increased brain-protective chemicals (BDNF and NGF) after a stroke-like injury was induced (neurotrophin study). Large-scale gene studies in rats showed Semax adjusts hundreds of genes related to inflammation and blood vessels after brain injury (BMC Genomics 2014; transcriptome study). It also reduced certain damage-related proteins in rat brains after simulated stroke (Int J Mol Sci 2021) and affected how a protein linked to Alzheimer's clumps together in lab dishes (study).
