AICAR is a small lab-made molecule, not a peptide (chain of amino acids). It's actually a natural stepping-stone chemical the body already makes in tiny amounts while building purines (building blocks of DNA and RNA). Scientists isolated and use it in research as a way to trick cells into thinking they're low on energy.

Mechanism under study

Imagine a fuel gauge inside every cell called AMPK — when it senses low fuel, it flips the body into "energy-saving and burning mode," telling cells to burn fat, take in more sugar for energy, and build more of the tiny power plants inside cells (mitochondria). AICAR fools this fuel-gauge system into switching on, even when the cell actually has plenty of fuel — which is why it's nicknamed an "exercise mimetic," because it flips on some of the same switches that real exercise does, without the exercise (USADA).

Human evidence summary

AICAR (under the name acadesine) has actually been tested in real human trials — but for heart surgery patients, not weight loss. A large phase 3 trial called RED-CABG tested it in people having coronary artery bypass surgery, but the trial was stopped early because it wasn't showing enough benefit (this trial's specific results weren't confirmed in detail here). Importantly, no human trial has ever tested AICAR specifically for weight loss, fat loss, or body shape changes — any use for that purpose is not backed by real clinical research in people.

Preclinical evidence summary

In mouse studies, sedentary (non-exercising) mice given AICAR for several weeks showed improved running endurance, which is part of why it earned the "exercise in a bottle" reputation.
