Thymosin Alpha-1 (often shortened to TA1 or Tα1) is a naturally occurring, 28-amino-acid piece of protein first discovered because it's the ingredient in thymus tissue that restores immune function in mice whose thymus was surgically removed. Its official drug name is thymalfasin, sold under the brand name Zadaxin (Vitam Horm 2016; DrugBank DB04900). Unlike most peptides in this batch, this one has real, approved medical use in dozens of countries.

Mechanism under study

Rather than working through one single "lock and key," Thymosin Alpha-1 nudges many parts of the immune system at once. It switches on immune cells called dendritic cells (the immune system's "scouts" that identify threats) through sensors called Toll-like receptors, which then triggers a cascade of immune activity (Vitam Horm 2016). It also helps infected cells display more "warning flags" on their surface so the immune system can spot and attack infected or abnormal cells more effectively, which is part of why it's sometimes combined with other immune-boosting treatments (Ann N Y Acad Sci 2007).

Human evidence summary

This peptide has real randomized clinical trials behind it, which is rare in this batch. A combined analysis of 7 trials (over 1,100 patients) with hepatitis-B-related liver scarring found that adding Thymosin Alpha-1 to standard antiviral treatment improved short-term response and lowered side effects compared to the antiviral drug alone — though the extra benefit faded by about a year later (BMC Gastroenterol 2020). However, a large, well-designed, placebo-controlled trial in over 1,100 sepsis patients (a life-threatening blood infection) found it did NOT reduce deaths, and even hinted at possible harm in patients under 60 — a genuinely disappointing and important negative result (BMJ 2025). In COVID-19, a combined analysis of 8 studies found lower death rates with Thymosin Alpha-1, but the studies varied so much from each other that scientists say more rigorous trials are still needed before drawing firm conclusions (Inflammopharmacology 2023).

Preclinical evidence summary

In lab animal studies, Thymosin Alpha-1 showed benefits against several cancer types (including lung cancer, leukemia, and melanoma models) and helped fight metastasis (cancer spreading) in combination with other immune-boosting treatments and chemotherapy (Ann N Y Acad Sci 2007).
