Mechanism under study

Here's something surprising: too much parathyroid hormone all the time actually breaks bone down, but tiny, once-daily doses of teriparatide flip the script and build new bone instead. It's like the difference between constantly nagging someone (which wears them down) versus a quick daily reminder (which motivates them to build something). This once-a-day dosing pattern triggers bone-building cells to activate, while also affecting how your kidneys handle calcium and phosphate (StatPearls).

Human evidence summary

In a major clinical trial of over 1,600 women with osteoporosis who'd already had a spine fracture, those on daily teriparatide injections had far fewer new spine fractures compared to those on a placebo (fake treatment): about 5% got a new fracture on the lower dose versus 14% on placebo. Bone density in the spine went up by about 9 percentage points (Neer et al., NEJM 2001). Nausea and headache were the most common downsides.

Preclinical evidence summary

In rats given the drug over their entire two-year lifespan at various doses, some developed a type of bone cancer called osteosarcoma. This raised concern initially, but 15 years of monitoring people who actually take the drug hasn't shown the same cancer risk in humans (StatPearls).
