Human evidence summary

The clearest human trial was a small, 12-patient crossover study (where each patient got both the real drug and a placebo at different times, in random order) in people with Barth syndrome. It found SS-31 was NOT better than placebo for walking distance or fatigue scores, though people's leg muscle strength did improve later during an open-label follow-up phase where everyone knew they were getting the real drug (FDA label). A larger, 218-person trial (called MMPOWER-3) in a different condition — mitochondrial myopathy — found no meaningful benefit at all on walking distance or fatigue after 24 weeks, a clear negative result (Karaa et al., Neurology 2023). Despite mixed results, the FDA approved it in 2025 based on the muscle strength improvement seen later in the Barth syndrome study, though this was controversial — reportedly, nearly a dozen FDA reviewers were against approving it (Reuters, Nov 2025).
