Semaglutide is a lab-made peptide sold under brand names like Ozempic, Rybelsus, and Wegovy. It's a copy of a natural hormone your gut makes called GLP-1, but it's been chemically tweaked so it lasts much longer in the body — natural GLP-1 breaks down in minutes, but semaglutide is designed to work for about a week per dose (IUPHAR/BPS peptide dataset; Min et al., 2025 PK review).
Mechanism under study
Your gut naturally releases a hormone called GLP-1 after you eat, which tells your brain "I'm full" and slows down digestion. Semaglutide copies this hormone but sticks around much longer, so it keeps sending that "full" signal to your brain for days instead of minutes. It works on brain areas that control hunger and cravings, and it also slows down how fast your stomach empties, which is part of why people eat less and lose weight (Papakonstantinou et al., 2024 review).
Human evidence summary
In a landmark study of nearly 2,000 people over 68 weeks, people taking semaglutide lost about 15% of their body weight on average, compared to only about 2% for people on a fake shot — and more than half of people lost 15% or more of their weight (NEJM 2021). In an even bigger heart-focused study of over 17,000 people followed for over 3 years, semaglutide lowered the chance of serious heart problems by 20% compared to placebo (NEJM 2023).
