LL-37 is a piece cut from a larger natural human protein called hCAP18, and it's part of your body's built-in germ-fighting system (called the innate immune system). It's sometimes given the drug-development nickname "ropocamptide" (Promore Pharma). It's made of 37 amino acids folded into a shape that likes both water and fat, which is part of why it can poke holes in germ cell walls.
Mechanism under study
Picture LL-37 as tiny daggers that punch holes in bacteria, fungi, and other germs' outer walls, killing them directly. But it does more than that — it also acts like a messenger, calling in immune cells, helping new blood vessels grow, and encouraging skin cells to move and multiply to close up wounds (Duplantier & van Hoek 2013). People with higher natural LL-37 levels in their blood tend to heal leg ulcers faster.
Human evidence summary
Results in real trials are genuinely mixed. A small first trial in 34 people with tough-to-heal leg ulcers found that a certain dose of topical LL-37 sped up healing quite a bit compared to placebo (a fake treatment), though a higher dose didn't work as well (Grönberg et al. 2014). But the bigger, more important follow-up trial with 148 people — called HEAL LL-37 — failed to show any real benefit for the whole group; only a smaller sub-group with larger wounds seemed to improve, and that kind of after-the-fact finding is less trustworthy (PMID 34687253). A separate trial on diabetic foot ulcer cream found faster healing but no actual drop in inflammation markers (Arch Dermatol Res 2023). Bottom line: the biggest, best-designed trial did NOT clearly prove it works.
