Hyaluronic acid isn't actually a peptide at all — it's a totally different kind of molecule called a glycosaminoglycan (say: gly-co-sam-in-o-gly-can), which is basically a long chain of sugar molecules linked together, not a chain of amino acids like a real peptide. It naturally exists in your joints, skin, and eyes, where it acts like a slippery cushion. For medical use, it's manufactured in different sizes (weights), and some versions are chemically "cross-linked" (bonded together more tightly) to make them last longer in the body (Cartilage 2021).

Mechanism under study

Think of hyaluronic acid like the oil that keeps a rusty door hinge moving smoothly — when injected into a joint like the knee, it's supposed to make the joint's natural fluid slippery and cushiony again, easing friction between bones. For a long time, regulators thought it worked purely as a physical lubricant, like grease, rather than through a chemical/biological pathway. But the FDA has now said the evidence suggests it might also work through actual chemical activity in the body, not just mechanical cushioning (Federal Register 2018).

Human evidence summary

Human studies have been extensive but the results are mixed and a bit disappointing overall. A major review found that hyaluronic acid injections into the knee might reduce pain scores in the short term, but they did NOT meaningfully improve function or stiffness on more detailed scoring systems (Pharmaceuticals 2024). When scientists looked only at the highest-quality studies, the benefit was so small it barely counted as meaningful (RMD Open 2015). It clearly beat plain saltwater injections in some FDA studies (HYMOVIS trial), but one major group of orthopedic doctors (AAOS) says don't bother using it routinely for knee arthritis, while another group (VA/DoD) gives it only a weak thumbs-up. Nobody has good evidence it works well outside the knee.

Preclinical evidence summary

The existing research summary does not describe any animal studies for hyaluronic acid — its evidence comes almost entirely from human trials and device-approval testing.
