Research overview
CagriSema combines two lab-made hormone-mimicking peptides: cagrilintide, which copies a natural hormone called amylin, and semaglutide, the same active ingredient found in Ozempic and Wegovy. Research versions typically mix them 1:1 by weight, matching the 2.4 mg/2.4 mg ratio Novo Nordisk uses in its official drug trials (REDEFINE 2, NEJM 2025).
Mechanism under study
Semaglutide works on the GLP-1 pathway, a hormone system that tells your brain you're full. Cagrilintide works on a different but related pathway (amylin) that also affects fullness and how fast your stomach empties. The idea is that hitting two separate 'I'm full' signals at once produces more weight loss than hitting just one. Unlike most peptide blends, this rationale isn't just a guess — it's backed by real combination trials on the actual manufactured drug.
Human evidence summary
This is one of the best-studied peptide combinations in existence. A large Phase 3 trial called REDEFINE 2 found people lost 13.7% of their body weight on average compared to 3.4% on placebo over 68 weeks in people with obesity and type 2 diabetes (NEJM 2025). Another trial, REIMAGINE 2, found the combination beat semaglutide alone (Novo Nordisk). Important catch: all of this strong evidence is for Novo Nordisk's official manufactured product — not for research-chemical vials bought online, which have unverified purity and contents.
