Research overview

ARA-290, also known as cibinetide, is an erythropoietin-derived peptide designed to activate tissue-protective signaling without stimulating red-blood-cell production in the way full erythropoietin can.

Research has explored inflammatory pathways, small-fiber neuropathy, tissue protection, and repair mechanisms. Evidence differs substantially across disease models and clinical settings.

Human evidence summary

Two small but real clinical trials exist. In the first, 22 people with sarcoidosis-related nerve damage (small fiber neuropathy) got either ARA-290 or a placebo through an IV for 4 weeks; the treated group showed meaningful improvement on a nerve-symptom questionnaire, but pain and fatigue scores improved about equally in both groups — meaning it didn't clearly beat placebo on those specific measures (Heij et al. 2012). The second trial, in 48 people with type 2 diabetes, gave daily shots for 28 days and found modest improvements in blood sugar control (HbA1c), pain scores, and triglycerides, plus improved nerve fiber density — but only in a specific subgroup, not across everyone (Brines et al. 2015). Both studies are small and haven't been confirmed by larger trials.

Preclinical evidence summary

In rodent studies, ARA-290 reduced pain hypersensitivity (called allodynia, where normally painless touch feels painful) and reversed nerve damage related to diabetes (Heij et al. 2012). This preclinical animal work is what originally supported moving into human testing.
