Alprostadil is not a peptide at all — it's a lab-made version of a natural body chemical called prostaglandin E1, a type of fatty signaling molecule (not a chain of amino acids). It's sold under names like Caverject, Edex, MUSE, and Prostin VR Pediatric, and is used for very different purposes depending on the form: helping men get erections, or keeping a crucial blood vessel open in newborn babies with certain heart problems (Prostin VR label).
Mechanism under study
Think of alprostadil as a chemical relaxant for blood vessel walls. It attaches to specific docking spots (receptors) on cells and tells the muscles lining blood vessels to loosen up, letting more blood flow through. In the penis, this widens the blood vessels and relaxes tissue, producing an erection. In newborns, it keeps a natural blood vessel (that should normally close after birth) open a bit longer, which can be lifesaving if the baby has certain heart defects (Caverject label).
Human evidence summary
In three big multi-center trials, injecting alprostadil directly into the penis produced a clear dose-response effect (meaning higher doses worked better, up to a point) and helped men regardless of the underlying cause of their erectile problems (NEJM 1996). A different delivery method — a small pellet inserted into the urethra (the tube urine passes through) — was also tested in randomized trials and found effective (NEJM 1997; Br J Urol 1998). For newborns, decades of real-world clinical use back its ability to keep the ductus arteriosus (a blood vessel babies need open for certain heart defects) from closing (Prostin VR label).
Preclinical evidence summary
Some early lab work showed the drug could relax human penile tissue samples in a dish, and separate experiments in monkeys showed it increased blood flow to that area in a dose-dependent way (Caverject label).
